My testicular volumes are: * Right: 8.1 cc * Left: 8.6 cc My semen analysis shows: * Count and motility are very good * But morphology is only 2% (Teratozoospermia) Hormones (FSH, LH, Testosterone) are normal. Symptoms: * Mild pain started on right side around end of March * Now mild/moderate pain on both sides, tolerable 1.In your experience, how many Grade 3 bilateral varicocele patients did NOT undergo surgery/embolization and still maintained good fertility long-term? 2.With my current reports, would you suggest observation or treatment? 3.How concerning is 2% morphology when count and motility are strong? 4.Can morphology improve naturally with lifestyle, diet, and supplements? 5.Can low testicular volume improve after treatment? 6.If pain stays tolerable and semen remains stable, can surgery be avoided safely? 7.If treatment is needed, what gives better long-term fertility outcomes: microsurgery or embolization? 8. Best clinics or Hospitals in Pune or Gwalior for this ?

Your semen analysis parameters are normal except morphology which you are already aware of. Your testicular volume is slightly on the lower side of normal range but not low. 2 things should be kept in mind regarding varicocele surgery, 1st surgery is done in symptomatic cases and 2nd in cases of male infertility where sperm parameters are abnormal but surgery doesn't warrants fertility. I'll try to answer your questions now. 1. Surgery for varicocele doesn't warrants good fertility. Many grade 3 patients doesn't undergo sugery. Need of surgery is determined by above 2 important factors. 2. Observation or treatment can't be decided just by looking at reports. If you have symptoms or If you are trying a baby despite having unprotected sex for 1 years then surgery may help you. An urologist will help you in this case. 3. Not very concerning as your other parameters which are more important is normal 4. Definitely yes 5. Not much 6. Yes this is what I told you in those 2 things 7. Micro surgical is gold standard but not done at all the places 8. Urokul is a legendary dedicated urology centre in Pune. Consult there. Hope this helps
