Your report shows subclinical hypothyroidism where  T3,T4 Are normal and TSH.is high.Thyroxine is required only if you have any symptoms related to hypothyroidism or TSH level is above 10. You need to recheck thyroid function tests after 3 months to monitor. Your report also shows low vitamin d ,take 60000 iu of vitamin d once a week for at least 15 weeks.
