The report sounds scary, plz help us understand if it includes 3 serious issues? Does it mean there are 3 issues at the same time- stones, cyst and something wrong with cervix also?

First- dont panic, no need to worry. All these are manageable
Regarding usg reports-
It suggests
Cholelithiasis( stones in gallbladder) - seem asymptomatic at present as your prescription describes a lower abdominal pain which is not a usual finding with gallstones. Anyways a stone > 1 cm size does require surgery and you should plan it electively rather than waiting for it to become symptomatic at which stage surgery can become difficult owing to inflammation although still feasible and best option.
Regarding PMOS/cervicitis- a hormonal panel and appropriate antibiotics and some other medications based on a gynaecologist advice will give you relief and control over your symptoms.
Next Steps
Visit a General Surgeon/ GI surgeon ( both are good, no need for confusion) for Cholelithiasis and plan surgery electively
For other two- schedule a visit to Gynaecologist

This report describes three distinct, very common conditions that are highly manageable and usually not immediately dangerous.
Regarding Cholelithiasis-- If they are not causing pain (biliary colic), nausea, or vomiting after eating fatty foods, they are often completely harmless and require no treatment other than occasional monitoring. If symptomatic, it should be removed. Please consult a GI Surgeon for this.
You should schedule a follow-up appointment with your gynecologist to discuss a management plan for the three gynecological issues. They will likely recommend a follow-up ultrasound for the cyst and check to see if you need any medication for the cervix.

Hello. 1st of all. YouR reports are showing 2 different departments interventions.
As per your symptoms in the opd prescription. You are having lower abdomen pain probably due to pelvic inflammatory disease. The antibiotics given will help.
And for Pmos and the hemorrhagic cyst. Please consider talking to your gynae
For gall bladder stone please get a referral in general surgery dept. Since the stone is 11mm you need a gall bladder removal soon

Your report does show three separate findings together, a gallbladder stone, a left ovarian cyst with what's called PMOS, and cervix inflammation. These affect different organs and aren't the same issue.
Next Steps
The report itself recommends a hormonal panel and clinical correlation. Please book that follow-up with your gynaecologist to go through it in person.
