Tetrabenazine

Description

Tetrabenazine is a monoamine depleter used in the treatment of abnormal, jerky, involuntary movements (chorea) associated with Huntington's disease, an inherited condition that results in the death of brain cells. This medicine is not recommended for use in patients below 18 years of age.

Allergy

Monoamine oxidase inhibitors (MAOI)

This medicine is not recommended for use in patients who have taken monoamine oxidase inhibitors within the last 14 days due to the increased risk of serious adverse effects.

Active Suicidal Tendency

This medicine is not recommended for use in patients with an active suicidal tendency or inadequately treated depression due to the increased risk of worsening of the patient's condition.

This medicine is not recommended for use in pregnant women unless absolutely necessary. Discuss all the risks and benefits with your doctor before using this medicine.

Breast-feeding

This medicine is not recommended for use in breastfeeding women unless absolutely necessary. All the risks and benefits should be discussed with the doctor before using this medicine. Your doctor may advise you to discontinue the medicine or to discontinue breastfeeding based on your clinical condition.
General warnings

Suicidal thoughts

This medicine may increase the risk of suicidal thoughts. Hence, it is contrainidicated in patients with active suicidal tendenices and an uncontrolled depression.

Driving vehicles and operating machinery

This medicine should be used with extreme caution due to the increased risk of side effects such as dizziness, fainting, and drowsiness. It is advised that you do not perform any activities that require high mental alertness such as driving a vehicle or operating machinery during treatment with this medicine if you experience any of these symptoms.

Dysphagia

This medicine should be used with caution in patients predisposed to dysphagia due to the increased risk of worsening of the patient's condition. Appropriate dose adjustments or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

Hyperprolactinemia

This medicine may elevate serum prolactin concentrations in some patients and therefore, it should be used with caution in patients with breast cancer. Appropriate laboratory testing, dosage adjustments, or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

Hypotension and orthostatic hypotension

This medicine may induce postural dizziness and syncope (fainting on getting up too quickly from a sitting or lying position). Therefore, it should be used with caution in patients with hypotension and orthostatic hypotension. Appropriate patient education, dose adjustments, or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

Use in pediatrics

This medicine is not recommended for use in patients below 18 years of age since the safety and efficacy of use are not clinically established.

Poor or extensive CYP2D6 metabolizers

Patients who require higher doses of this medicine should be first tested and genotyped to determine if they are poor or extensive metabolizers of CYP2D6. Appropriate dose adjustments should be made for such patients based on their clinical condition for the best possible therapeutic effect.

Neuroleptic Malignant Syndrome (NMS)

Use of this medicine may cause neuroleptic malignant syndrome (NMS) in some patients. This condition is characterized by symptoms such as a very high fever, muscle rigidity, altered mental status, and autonomic instability. Appropriate corrective measures, dose adjustments or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

Alcohol consumption

Consumption of alcohol is not recommended during treatment with this medicine due to the increased risk of serious adverse effects.

Parkinsonism

This medicine should be used with caution in patients with parkinsonism due to the increased risk of worsening of the patient's condition. Appropriate dose adjustments or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

Missed Dose

Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose. Do not double your dose to make up for the missed one.
