Description
Telmiprime AM (5/40 mg) Tablet is a combination of Amlodipine and Telmisartan. It is an anti-hypertensive medicine used for the treatment of high blood pressure. This medicine works by relaxing the blood vessels, making the blood flow easily, thus lowering blood pressure. This will reduce your risk of having a heart attack or a stroke by making it easier for your heart to pump blood around your body. Telmiprime AM (5/40 mg) Tablet shows some common side effects like swelling in your ankles or feet, dizziness, back pain, headache, sleepiness, nausea, diarrhoea and stomach pain. Most of these are temporary and usually resolve with time. Telmiprime AM (5/40 mg) Tablet is also known to cause sleepiness and dizziness, so you should not drive or do any activity that requires mental focus after taking medicine. Avoid consuming alcohol with this medicine as it can worsen dizziness. Telmiprime AM (5/40 mg) Tablet can be taken with or without food. The dose will be decided by your doctor depending on your blood pressure levels. Avoid abrupt discontinuation of the treatment as your blood pressure may increase if you do so. Telmiprime AM (5/40 mg) Tablet is not recommended for use if you are allergic to it. If you have any pre-existing kidney or liver problems, or other medical conditions, inform your doctor about them. Inform your doctor about all medicines and supplements you are taking as this medicine can interact with them and can cause side effects. Telmiprime AM (5/40 mg) Tablet is not recommended for use in children below 18 years of age. Consult your doctor if you are pregnant, or are breastfeeding.

Allergy

Avoid taking Telmiprime AM (5/40 mg) Tablet if you have a known allergy to amlodipine, telmisartan or any other component of this medicine. This could lead to symptoms like skin rash, itching/swelling (especially of the face/tongue/throat), breathing difficulty, hives (painful eruptions that itch and sting), etc.

Aliskiren

Aliskiren is the first in a class of drugs called direct renin inhibitors used for the treatment of hypertension ( increase in blood pressure). Coadministration of aliskiren and Telmiprime AM (5/40 mg) Tablet is not recommended as it may increase the risk of hyperkalaemia (high levels of potassium in your blood), which in severe cases can lead to kidney failure, muscle paralysis, irregular heart rhythm, and cardiac arrest. This risk is especially higher in people with diabetes and kidney problems.

Pregnancy

Telmiprime AM (5/40 mg) Tablet should not be used by pregnant women as it may cause harm to the developing foetus. Consult you doctor if you are pregnant or planning a pregnancy.

Breast-feeding

Telmiprime AM (5/40 mg) Tablet is not recommended for use in breastfeeding women as may pass into breastmilk in some amounts. Consult your doctor if you are breastfeeding while taking this medicine.
General warnings

Hypotension

Telmiprime AM (5/40 mg) Tablet should be used with caution if you have aortic stenosis ( narrowing of the valve of the aorta, the largest artery to branch off from the heart). This medicine may decrease the functioning of the heart and lead to extremely low blood pressure and heartbeat.

Use in children

Telmiprime AM (5/40 mg) Tablet is not recommended for use in children below 18 years of age since the safety and efficacy of the use of this medicine are not clinically established.

Hyperkalaemia

Hyperkalaemia (increase in the blood potassium levels) may occur after taking Telmiprime AM (5/40 mg) Tablet. It may result in kidney failure, muscle paralysis, irregular heart rhythm, and cardiac arrest. It is important to maintain adequate fluid intake during treatment with this medicine

Renal artery stenosis

Renal artery stenosis is the narrowing of one or more arteries that carry blood to your kidneys. Telmiprime AM (5/40 mg) Tablet should be used with caution if you have renal artery stenosis as this medicine will alter the flow of the blood in these blood vessels, which may lead to improper filtering of the blood. Your blood urea and serum creatinine levels will be closely monitored by your doctor.

Primary aldosteronism

Primary aldosteronism occurs when your adrenal glands produce too much of a hormone called aldosterone. This hormone regulates salt and water levels in the body. Telmiprime AM (5/40 mg) Tablet is not recommended for use if you have primary hyperaldosteronism since it may affect the salt and water balance in your body and worsen your condition.

Driving and operating machines

Telmiprime AM (5/40 mg) Tablet has a moderate effect on the ability to drive or use machines. If you experience side effects such as dizziness or drowsiness, avoid performing activities that require high mental alertness like driving or operating machines.

Liver impairment and biliary obstruction

Telmiprime AM (5/40 mg) Tablet is not recommended for use if you have severe liver problems. This medicine is broken down and absorbed in the liver. If you take this medicine while you have pre-existing liver problems, it will worsen your condition further.

Kidney impairment

Telmiprime AM (5/40 mg) Tablet should be used with caution if you have severe kidney problems. This medicine is filtered and excreted through the kidneys. Using this medicine while you have kidney problems will damage your kidneys and worsen your condition.

Missed Dose

If you forget to take the dose of Telmiprime AM (5/40 mg) Tablet at a given time, then take it as soon as you remember. If it is almost the time for your next dose, skip the missed dose. Do not double your dose to make up for the missed dose.

Overdose

Never take more than the dose prescribed by your doctor. If you have intentionally or accidentally consumed more than the prescribed amount of Telmiprime AM (5/40 mg) Tablet, then you might feel headache, confusion, fever, change in the heartbeat, nausea, vomiting etc. Contact your doctor immediately for further guidance.

Cardiogenic shock

Telmiprime AM (5/40 mg) Tablet is not recommended for use when you have cardiogenic shock (a life-threatening condition where your heart abruptly stops pumping blood). Using this medicine will further decrease your heart’s ability to pump blood and reduce your blood pressure and heart rate drastically. This can be fatal.

Congestive heart failure

Congestive heart failure is a chronic progressive condition that affects the pumping power of your heart muscle. Telmiprime AM (5/40 mg) Tablet should be used with caution if you have congestive heart failure, as it will further reduce your heart’s ability to pump blood leading to very low blood pressure and slow heartbeat.

Coronary Artery Disease

Telmiprime AM (5/40 mg) Tablet should be used with extreme caution if you have coronary artery diseases (a condition where the blood vessels of your heart are damaged, mainly due to build-up of plaque) as it may decrease your heart rate. If this medicine is prescribed in such conditions, close monitoring of the heart function is required. Your doctor will suggest tests to closely monitor your heart function while you are on this medication

Angioedema

Angioedema is the rapid swelling of the area beneath the skin because of fluid retention. Telmiprime AM (5/40 mg) Tablet has to be used with caution as it may cause your body to retain excess water, which could lead to fluid retention and swelling in certain places like the face, eyes, lips, tongue or legs. Do not use the medicine if you have a history of angioedema related to previous therapy or if you have hereditary angioedema. Inform your doctor about such conditions.
Food interactions
Consumption of grapefruit juice is not recommended during treatment with Telmiprime AM (5/40 mg) Tablet as it can increase the absorption of this medicine. This could lead to excessive lowering of blood pressure or worsen side effects.
Using Telmiprime AM (5/40 mg) Tablet together with multivitamins containing minerals may decrease the effects of this medicine. Take these two medicines with at least a 2-hour interval Avoid a high potassium diet or supplements containing potassium during treatment with Telmiprime AM (5/40 mg) Tablet. It can cause an increase in the potassium level in your blood leading to symptoms such as weakness, irregular heartbeat, confusion, tingling of the extremities, or feelings of heaviness in the legs.

Telmiprime AM (5/40 mg) Tablet may be taken with or without food. Swallow the tablet whole with a sufficient amount of water. Do not crush, break or chew the tablet. The dose prescribed is based on your situation thus, avoid taking more or less dose than that prescribed by your doctor. You should not stop taking the medicine abruptly without your doctor's consultation as it can cause withdrawal symptoms. You should follow a balanced diet and exercise regularly to get the maximum benefit from the treatment. Avoid taking potassium-rich food as it may increase your potassium level in the blood. Also, avoid taking grapefruit juice with Telmiprime AM (5/40 mg) Tablet as it can increase the level of this medicine in your blood. Be careful while standing up from sitting or lying down position as this medicine may cause a sudden drop in your blood pressure which can make you dizzy. Telmiprime AM (5/40 mg) Tablet can cause swelling in your ankles and feet so try raising your legs when you are sitting. Consult your doctor if the swelling worries you. This medicine can cause sleepiness and dizziness so try to avoid activities that require mental focus, especially at the start of the treatment or until you know how this medicine will affect you.
