Description
Lomotil Tablet is used for the treatment of diarrhoea. It slows down the movement of food and water in the gut. This helps to reduce the number and frequency of loose stools. Lomotil Tablet is not recommended to treat diarrhoea caused by infection by bacteria or other organisms. Lomotil Tablet contains Diphenoxylate and Atropine. Diphenoxylate may be abused by taking it in higher doses due to its physical dependence. A small amount of atropine is added to this medicine to prevent its abuse. Atropine causes unpleasant effects in individuals who take larger than normal doses of this medicine and discourages abuse. Lomotil Tablet may cause side effects such as constipation, dry mouth, indigestion, dizziness, drowsiness or difficulty in urination. You can take this medicine four times a day or as directed by your doctor. The dose is gradually reduced so that you need to take a dose enough to control the diarrhoea. Do not take it for a longer period than prescribed.

    Lomotil Tablet starts its action within 1 hour of administration. Your diarrhoea symptoms will improve within 48 hours of treatment with this medicine.

Allergy

Avoid taking Lomotil Tablet if you are previously allergic to it. Seek immediate medical attention if you notice any symptoms of allergic reactions such as skin rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, breathing difficulties, etc.

Pseudomembranous colitis

Pseudomembranous colitis is the swelling of the intestine that happens when there are too many bacteria in the gut mainly Clostridium difficile or C. diff. It can occur as a side effect of antibiotic use. This may lead to frequent watery diarrhoea. Lomotil Tablet is not recommended to treat diarrhoea caused by pseudomembranous enterocolitis.

Inflammatory bowel disease

Inflammatory bowel disease is a group of intestinal disorders that cause prolonged swelling of the gut mainly the intestines. Crohn's disease and ulcerative colitis are the two forms of inflammatory bowel disease. Lomotil Tablet is not recommended to use in people with inflammatory bowel disease due to the risk of worsening of the condition.
Warnings for special population

Pregnancy

Lomotil Tablet is not recommended for use in pregnant women because it can slow down the breathing of newborn infants. If required, it should only be used upon your doctor's recommendation.

Breast-feeding

Lomotil Tablet passes into breast milk and hence not recommended for use in breastfeeding women. An alternate medicine may be preferred, especially while nursing a newborn or preterm infant. If required, it should only be used upon your doctor's recommendation.
General warnings

Infectious diarrhoea

Lomotil Tablet is not recommended for use in patients having infectious diarrhoea. The symptoms may worsen and get prolonged if this medicine is used in such cases. Any incidence of diarrhoea accompanied by the presence of blood or pus in stools and high fever should be promptly reported to the doctor.

Driving or operating machinery

Lomotil Tablet may cause dizziness, confusion, or drowsiness and hence do not perform any activities such as driving a vehicle or operating machinery if you experience any of these symptoms.

Use in children

Lomotil Tablet is not recommended for use in children less than 12 years of age since the safety and efficacy of use are not clinically established.

Dehydration and electrolyte imbalance

Diarrhoea can cause severe dehydration (loss of body fluid) and electrolyte imbalance. Lomotil Tablet cause fluid accumulation in the intestine, which may further worsen dehydration. Rehydration and electrolyte replacement should be started before using Lomotil Tablet in patients with dehydration and electrolyte imbalance.

Withdrawal symptoms

Do not stop using Lomotil Tablet immediately because of withdrawal symptoms such as nausea, vomiting, stomach cramps, sweating or tremors (involuntary movement). Your doctor may reduce your dose slowly to avoid these symptoms. Individuals who have used this medicine for a long term or in high doses are at the risk of developing withdrawal symptoms.

Missed Dose

Take the missed dose of Lomotil Tablet as soon as possible. If it is almost the time for your next dose, skip the missed dose. Do not double your dose to make up for the missed dose.

Overdose

Seek emergency medical treatment or contact the doctor in case of an overdose with Lomotil Tablet.

Liver Disease

Lomotil Tablet gets metabolized (change into an active form) in the liver. Use this medicine cautiously in patients with liver diseases due to the risk of medication toxicity. A dose adjustment might be required in some cases.

Kidney Disease

Lomotil Tablet is filtered through kidneys and gets removed via urine. Therefore patients with kidney diseases might require a dose adjustment based on the clinical condition.

Hypertension

Lomotil Tablet can cause high blood pressure and hence use this medicine cautiously in patients with hypertension.

Heart Disease

Lomotil Tablet should be used with caution in patients with heart diseases such as tachycardia (increased heart rate), congestive heart failure, or coronary artery disease due to the risk of adverse effects on the heart.

How it works
Diphenoxylate works by reducing the movement of food and water from the stomach and intestine. The reduced bowel activity makes the food stay longer in the intestine resulting in increased absorption of water. Atropine helps to dry up body fluids and reduces muscle contraction in the intestinal wall. This helps to slow the movement of food from the intestines.
