It’s looking like skin tag (sentinel pile) which occurs due to fissure and not pile .Anal fissure occurs due to passing hard stool or straining while passing stool.
Get examined by general surgeon and surgeon will advise further treatment like ointment and Sitz bath , if you have constipation then use syrup for that as advised by doctor

Seems like an anal fissure or it can also be an underline hemorrhoid.
For now you can have some lifestyle changes -!that is increased dietary fiber , do not sit for long on the commode and have liberal 4-6 L of water everyday.
