Spotting but haven’t had period in year

Hi, I had my son September 2015 via c section then I got mirena put in that December. I ended up stop getting my period about a year ago( but every time around my period I get symptoms just no blood) and every time around my period time, around my incision hurts and sometimes to the point where it’s so painful it’s hard to stand. I recently started working out and my discharge has a little blood in it, idk if I should be worried or not.
