Portal hypertension and ascites.
I'm attaching my father's USG report which shows portal hypertension and mild ascites. He's having fever since 3 days and facing problems in passing stools, constipation kind of. And the stool is black in colour though he's not having any iron tables. Please go through the report and tell me how serious is the condition. Do we need to worry? Thanks.

Passing black colour stools means that some internal bleeding is happening which can happen in patients with  portal hypertension.  Please see you're doctor immediately since the source of bleeding  need to be found out. Don't delay  since he can have a massive  haemorrhage anytime.
