Hard mass around nipples

Asked for Male, 21 Years

For around 8 years, I have had puffy nipples, and hard mass around them. When I consulted doctor back then, I was told it was just puberty. But just a few days back, I had some pain in my right nipple. I am not sure, but I feel the hardened mass has spread a little bit too. This is frightening me. What could it be?
