Spinal Tuberculosis (TB) causes  pain due to the bacteria causing destruction of bone in Spine ..Pain is worse  on changing positions ..appropriate dose TB drugs if effective  taken daily would help slowly reduce the pain over 2-3months ..during that time should keep a watch on weakness in legs.. If pain is worsening while on TB drugs that suggests  maybe you are not taking proper dose or its drug resistant  TB Ideally i recommend  a biopsy before  starting TB drugs to check  if the medicines  are  going to be effective or not.
Next Steps
please share your MRI images   & biopsy reports alongwith any blood reports Share list of TB drugs which you are taking with dosage.. also share your bodyweight
Health Tips
painkillers  will help only if there is pain while you are lying in bed.. but you will  still experience pain on changing positions.. Keep some patience as its just been 20 days since you started TB drugs There's  a long battle ahead for the next one year .. Rgds Dr Bhavin Shial Consultant Spine  Surgeon Mumbai
