Pain in the legs at rest or while sleeping is unlikely to be due to varicose veins. It could be due to a large disc prolapse, in which case he should also have numbness and/or weakness in the legs. Another cause is deep vein thrombosis, which is associated with calf swelling with or without fever or breathlessness. Finally do rule out arterial insufficiency like Buerger's disease, especially if he is a chronic smoker.
