Your symptoms suggest that you are suffering from an anxiety disorder. Anxiety disorders are treatable with psychotherapy (CBT) and medicine (SSRIs and bezodiazepines). For sleep you should follow sleep hygiene practices ( I have given those in helpful tips session)

sleep hygiene practices: 1. fixed wake-up time 2. early morning exercise 3. No naps during daytime 4. No tea or coffee after 6:00pm 5. No mobile or TV screen before bed. 6. comfortable bedding for sleep 7. Use bedroom for sleep only ( No TV or reading in bedroom) 8. don't stay in bed for more than 30 minutes if you don't get sleep. come out of bed read a book till you feel sleepy. you can try it many times but don't spend time in bed trying to get sleep. 9. stay positive don't let your mind be occupied with negative thoughts about sleep.
