The HbA1c improving from 6.5 to 5.6 is excellent and reflects good overall sugar control over the past few months. However, the post-meal reading of 148 is slightly above the stricter pregnancy targets (ideally below 140 at 1 hour after meals). In pregnancy, blood sugar targets are tighter than outside pregnancy because elevated sugars affect the baby's growth. Pregnancy also increases insulin resistance progressively as it advances, meaning the same medication and diet may become insufficient over time — this is expected and the treatment plan often needs adjustment as the pregnancy progresses.
Next Steps
Please share these readings with her obstetrician and diabetologist at the earliest. Do not change the metformin dose on your own. Frequent glucose monitoring and close follow-up are very important in a diabetic pregnancy. Please consult me directly on Practo for a detailed evaluation.

She is having high sugars. If sugars are not subsiding with metformin, she needs to take insulin. That's the treatment of choice in pregnancy. Because high sugars can cross placenta and influence foetus as well. Though HbA1c has come down still her PP sugars are high.
Next Steps
Do a sugar chart and feel free to consult me on practo

Slight fluctuations can occur in blood sugar levels, due to meals, meal timings, exercise,  stress. So far as it remains in normal range, there is nothing to worry.
