TREATMENT OF SALMONELLA TYPHI OF GB

What course of antibiotics is given when Typhoid bacteria/ Salmonella Typhi enters the GB and starts residing in it. How is it possible that bacteria enters from jejunum into the Gall Bladder ? What different course of antibiotics is given to treat such complicated cases of typhoid ? Salmonella is showing drug resistance towards Cefuroxime, Cefixime , cedpodoxime , ciprofloxacin , Linezolid etc. Even blood culture report isn't able to reach a conclusion. What the antibiotic to treat such cases ?

If the above drugs are resistant then Meropenem should be tried as a next step. And Azithromycin should also be tried. Since blood culture is not positive diagnosis is still questionable!
