To get my blood/urine reports checked

Hi, i want to get my reports checked. Urine Nitrogen levels are quite down. Vitamine b12 and vitamine D is also very low. Please guide a suitable diet or medications to get these back to normal. Also, if this a cause of concern then please suggest the further steps to cure this. Attached is the snapshot of my report parameters.
