Repeat Tsh and free T4 after 1 week . As der is decrease in tsh levels just monitor values to initiate therapy . If tsh decreasing with normal range f t4 . No need to start therapy

Current biochemical data does not meet the threshold to initiate thyroid hormone replacement therapy. Assessment & Plan: Current Status: Mild, residual TSH elevation at 25 days of life. Action Required: Repeat serum TSH and Free T4 (FT4) in approximately 7 days. Prognosis If TSH continues to normalize and FT4 remains stable, clinical observation will suffice. If TSH remains persistently elevated or FT4 falls below standard reference ranges, further diagnostic evaluation and therapeutic intervention will be initiated.

Good thing...  your kid TSH is in decreasing trend so repeat again after  week
