Thyroid swelling

69-year-old female with a long-standing multinodular goiter. Ultrasound (7 months ago) showed a markedly enlarged left thyroid lobe (11 cm) with a 5.5 × 3.9 cm TI-RADS 3 solid nodule and a 1.3 × 1 cm TI-RADS 4 peripherally calcified nodule. Trachea is deviated to the right. No cervical lymphadenopathy or retrosternal extension. Previous FNAC/biopsy was benign. TSH: 0.87 mIU/L. She has been taking Carbimazole 5 mg once daily and Propranolol LA 20 mg once daily for the past 7 months. She currently prefers to avoid surgery and doctor advised her on regular check up. Is it okay for her to continue the medicine or surgery the only option. Please reply
