Serum lithium level is usually checked 5–7 days after starting lithium or after any dose change, and the blood sample should be collected about 12 hours after the last dose. Once the therapeutic level is achieved and the dose remains stable, monitoring is generally done every 3 months initially and then every 6–12 month. Kidney function and thyroid function tests should also be monitored periodically while on lithium. Please follow me for psychiatrist's specific instructions regarding the timing of the test.
