Teeth Yellowing and Browning

My teeth are yellow (  the front side) not simultaneously but some of them and the above teeth which are above(incisors only) Form a Plaque(yellowish white) And the back side of teeth have formed some tartar like stuff yellow brown from gum of teeth.Pls help to sort out.
