Tb treatment

My mother was diagnosed with tb doctor started att treatment not according to her weight her fever resolved in 3-4 days but alt/ast rise to 440/660 .so all medicine were stopped and shifted to S/L/E regimen after 10-11 days when her enzymes level were below 100 they removed S and added isoniazid/ levo/ethambutol but this too was in high dose after one week rifampicin was added that too in high dose lft after 3 days were normal so pyrazinamide was added on adding pyrazinamide her alt/ast rose to 560/790 then I was told to stop all medicine I requested the emergency doctor not to stop but start alternative regimen S/L/E was started on starting S/L/E she started having fever again which slowly increased daily for one week so I got her chest x ray done which was showing her tb has increased at alt/ast 138/120 she was again started on att but this time according to weight her fever resolved, but she had body pain and itching is there any chance of drug resistance tb in her case

Drug resistance wont happen as soon as you stop medications. Pyrazinamide is known to cause liver injury. in this case we can start with half the dose as per her wt band of pyrazinamide while continuing others and then slowly increase the dose every 3 days.

Stopping and starting of Att is common in certain conditions. Pl consult govt tb centre, where there is robust protocol is there. Pl consult the specialist there.

She is having allergic reaction ,need to stop and restart each drug at a time may be pyrazinamide might be the cause ,get uric acid levels done ,if drug found as culprit  regimen can be altered accordingly
