Swollen Lymph Node under chin
I am having swollen lymph node which has grown around 1.5 to 2 cm and have occasional cough, and doctor suspected this as TB and asked for Mantoux test and QuantiFERON-TB Gold which shows it is positive(both the reports attached). For better clarity doctor asked for ultrasound scan of neck which showed 3-4 lymphnodes. And then ultrasound guided FNAC where sample was not collected and properly and Lab asked for repeat procedure. Doctor said before FNAC itself that even if it is negetive result the lymph node needs excision. I would like to know if this is TB can't I get it solved with medication without excision. Please guide me on this.

From the test result you are infected with TB baccili.
Infection and disease are two different thing.
a positive Mantoux test shows you are infected but cant say you have active TB disease or not.
As you have enlarged LN and occasional cough , you may have active extra pulmonary TB.
a good lab tech should have been able to take a good sample from FNAC and that too under US guidence.
Next Steps
I suggest following, please.
1.Get Chest X ray done. PA view.if not done already.
2. if sputum can be produced by you, especially early morning - than ask sputum test by Genexpert. Its most reliable sputum test available in most major cities.
3. Get FNAC done from a good lab in your city. The cytological exam will show TB or otherwise.
I would advise no excision of LN at this moment, as if its enlarged by TB it can regress by medications if not big or discharging pus ( cold abscess) .
