Patient: Mother, Postpartum Day 8–9 (Breastfeeding). Issues: Fever 102°F, hard swollen left breast with redness above areola (suspected mastitis), cracked/tilted nipple causing latching failure and milk stasis. Note: Has a pre-existing benign disk/fibroadenoma in the left breast since childhood, aggravating duct blockage. Request: We are in a rural area and cannot travel with the newborn right now. Please review the attached photo and prescribe a safe breastfeeding-compatible antibiotic and dosage.

Your symptoms fit mastitis: a hard, red, swollen area above the areola with fever, caused by milk stasis from the cracked nipple and latching trouble. The old fibroadenoma in that spot can make the blockage worse. The photo shows redness and a crack near the nipple base, which matches what you've described. This usually needs an antibiotic safe for breastfeeding, started soon.
Next Steps
Since travel isn't possible, book a video consult with a gynaecologist or lactation consultant today so they can prescribe the right antibiotic for you. Keep breastfeeding or pumping from that breast often, since clearing the milk actually helps rather than making things worse. Warm compresses before feeds and paracetamol for the fever are safe in the meantime.
Health Tips
Go to the nearest hospital today if the breast develops a firm, fluid-filled lump, or if the fever keeps rising. That combination can mean an abscess needing drainage.
