Is it itchy? Does he have pain ? The pic is not very clear. Try to meet a dermatologist, it could be tenia(fungal infection) or allergy . Best is to meet a dermatologist.

Use Democalm lotion. Use mild cleanser like cetaphil bar soap and also a moisturizer like cetaphil moisturizing lotion. It looks like minor allergy to something, may be soap. Contact again if not better
