You are suffering from chronic tellogen effluvium Since your Vit B12 is deficient, you need to add more animal organ meat like liver and  kidneys, fish, cereals and eggs in your diet. You can start using Minoxidil 5%solution for your hair along with some hair supplements. Your hair fall will slowly stop in a couple of weeks time.
