From the history mentioned here, the patient is a known case of Rheumatoid arthritis.

Pain in joints of fingers is common presentation of rheumatoid arthritis and there can be relapses and also pain may increase when the treatment doses are reduced or stopped.
As required in rheumatoid arthritis I would ask you to continue medications which had reduced the pain initially.
For severe pain now you can take -
Tab Acenac SP twice a day
Tab HCQs twice a day
Any other medications (Folitrax/ steroid) if already prescribed initially by your doctor.
