Rashes on cheeks

Hi, My 4 month baby develops rashes on his both the cheeks. It also causes itching at times. We bathe him with only water and don't apply anything on his face. Please suggest how to reduce the rashes. Due to rashes, his cheeks have become rough to touch. The rashes become more and less prominent from time to time. Thanks.
