Hi, the swelling with pus sounds like it needs closer attention as antibiotics alone aren't helping yet. Keep the area clean, elevate the leg and apply warm compresses gently. A review to check if drainage or a change in treatment is needed would be worthwhile. Feel free to consult for further guidance.

There is a visible swelling, which as you are describing seems to be abscess, I.e. pus filled swelling. Along with your meds, apply sumeg ointment 3 times daily, which should settle it or make it burst within 2 to 3 days. If not, get incision and drainage done.

Looking at the image, there is visible swelling around the ankle with a small opening where the pus has been draining, which is typical of a skin abscess or soft tissue infection with a pus collection. Antibiotics alone are often not enough when there is a formed pocket of pus — the collection usually needs to be surgically drained (incision and drainage) by a doctor to allow proper healing. Completing the antibiotic course is still important, but the pus re-forming suggests the infection needs direct drainage.
Next Steps
Please visit a surgeon or surgical OPD urgently for proper drainage of the pus. Please consult me directly on Practo for a detailed evaluation.
