Prominent right Hillar shadow
Please check the X-ray report that I have attached with this question and suggest what I should do. Is it a matter of concern? Dry cough for the past 1 to 1.5 years. It sometimes gets better but then returns again. Shortness of breath / difficulty taking a full breath. I sometimes experience a feeling of heaviness in the chest.

A "prominent right hilar shadow" on a plain X-ray is a non-specific finding - it can simply be a normal or prominent pulmonary vessel or a technical/positional effect, but it can also mean enlarged lymph nodes from tuberculosis, sarcoidosis or infection. Given your dry cough with intermittent breathlessness and chest heaviness for 1 to 1.5 years, this needs proper evaluation rather than reassurance from an image alone; it is not an emergency, but it should not be ignored either. with breathlessness and chest heaviness does need proper evaluation rather than reassurance from an X-ray alone, and asthma or allergic airway disease, post-nasal drip and acid reflux are common treatable causes at your age.
Next Steps
Please see a pulmonologist or physician in person with the actual X-ray film. The usual next steps are a CECT chest (which settles whether the hilum is truly abnormal), spirometry with reversibility to check for asthma, a CBC with ESR, and TB testing including sputum if indicated. Long-standing dry cough with breathlessness is often asthma or allergic airway disease, which is very treatable once confirmed. Seek prompt care for coughing blood, unexplained weight loss, evening fevers or worsening breathlessness. A prominent right hilar shadow on a chest X-ray means the root area of your right lung appears larger or denser than normal, usually due to enlarged lymph nodes, blood vessels, or a mass.
Especially in India, and 1 to 1.5 years of cough, breathlessness, chest heaviness point towards TB.
Get a HRCT chest for better understanding of hilar shadow.
Sputum for AFB as well
