Hello, If it is about whether you have hemorrhoids ( piles) / Fistula , if you have pain or bleeding per rectum while passing stools, you have to undergo proctoscopy for clear visualisation of the anal verge and rectum. If you have been diagnosed with hemorrhoids/ fistula, you have to undergo sigmoidoscopy to rule out internal hemorrhoids or presence of other lesions. In case of not finding anything via either proctoscopy or sigmoidoscopy but symptoms persists, colonoscopy will be preferred as a diagnostic option.

For diagnosing piles, fissures, or fistulas, different tests serve different purposes: 1. Practoscopy (Proctoscopy) – This is a simple and effective test for diagnosing piles (hemorrhoids) and anal fissures. It involves inserting a small tube to examine the anal canal and lower rectum. 2. MRI – This is mainly used for complex cases of fistulas, especially to check for deeper infections or multiple tracts. 3. Colonoscopy – Typically not needed unless there are symptoms like bleeding higher up in the colon, unexplained weight loss, or suspicion of other bowel diseases. Best Approach: Start with a physical examination by a colorectal surgeon or proctologist. A proctoscopy is often enough for piles and fissures. If a fistula is suspected, an MRI fistulogram might be required.
