Persistent Anal Itching

I've had anal itching for over 2 months. It intensifies at night and scratching it causes pain and tenderness. I saw a dermatologist who gave me a cream called lulibet to apply locally. It hasn't helped much. The itching is driving me crazy and the resulting pain and tenderness are hard to bear.
