Pelvic floor surgery chronic tension

My muscles & spinchter aren't relaxing. & causing incomplete evacuation & cramps. I've undergone almost 4 anal surgeries. I follow every advise by doc. Still there's pain. I've been taking laxatives daily for 2 years. Cremaffin plus causes diarrhea daily. All night it causes stomach cramps & I end up not sleeping well. I recently realized, I had the habit of half squatting while peeing. Like bending a little & standing. Since 7 years I have never sat down to pee. To avoid UTI. Could that have caused muscle disorientation & tension leading to chronic pain? If yes, will it not become like before? please guide how to relax again like it used to be 7 years ago. Please🙏 😭 I've been hopelessly undergoing surgeries. Again doc has told surgery. I might die if I keep undergoing surgeries so often. I have other surgeries to undergo for other issues too. It got delayed as I've been undergoing this surgery repeatedly. lost too much weight & weakness coz of pain & surgeries.
