Painful urination

From last few days, pain and irritation is being  experienced by the patient while urinating. Also the opening of urinary tube (at the tip of penis) has become narrower. At times some kind of thick liquid oozes out of the urinary tube. Also there appears to be a collection of white color substance making a fine layer inside the tube, resulting in narrowing of the passage.
