Pain in rectum after bowel movements

Once every few months, I will have a bowel movement that, once released, is followed by a very painful sensation inside my rectum. It is so bad that I have to get off the toilet, lie on the ground, and focus on my breathing until the sensation passes. It only lasts a minute or so but is very painful. This has been happening since I was quite young (I am now 23).
