Hospital... Go to a medical centre who would define the cause of fever through blood tests and examination findings. Take opinion of second medical consultant if not helped by current treatment ( if that was advised by a doctor). I would recommend going to a senior consultant in a reputed hospital. Joint pains for 10 years can be tackled later on, but the reasons is most probably arthritis, type will be deducted by clinical examination and tests again.
