As per your history i need to examine the patient first only then I will be able to tell what is the reason, from history it's not sure if it's really rheumatoid or something else.So please show the patient to some experienced orthopaedic surgeon

From your description it seems that she is suffering both from Rheumatoid as well as Osteoarthritis. If ahe didnt get relief from steroid injections then the next option is to either get artificial gelly injection or Surgery. For Grade 2 even platelet rich plasma (PRP) i injection can be given. Any decision can be made only after examining her. Also her thigh pain may be most likely from a pinching of nerve in her back.
Next Steps
Consult with a doctor of pain mangement clinic
