Stage 3 osteoarthritis required knee replacement. But all that also depends whether he has pain on sitting or doing activity. Age of the patient? Share xrays if you can. Try all conservative measures before going in for surgery.

Total Knee replacement is the last resort for treatment of Osteoarthritis knee. How is his pain at present. Is he able to perform his routine activities. This surgery is a planned procedure. Depends on the condition of the knee.
Next Steps
If his activities have decreased than what they were previously, better to get operated. if he is able to do his routine work and bear with the pain, can take conservative treatment in the form of analgesics, exercises etc..
Health Tips
Avoid knee bending, sitting down, avoid heavy work etc..
