Need urgent second opinion

I would like a second opinion regarding my symptoms and evaluation so far. Since January, I have had a persistent dry, chesty cough along with intermittent air hunger, chest heaviness/pressure, and breathlessness. I often feel unable to take a satisfying deep breath and frequently sigh or yawn. Symptoms worsen at night, when lying down, after prolonged talking, laughing, brushing teeth, and sometimes after meals. There is occasional mild wheeze on bit forced exhalation. I also experienced prolonged hoarseness and voice fatigue last year. Initially there was some mucus, but the cough later became mostly dry and vibratory. No recurrent fever. ENT exam was normal. Tests showed normal PFT, FeNO 17 ppb, normal chest X-ray, and total IgE was also normal. The pulmonologist suggested possible Cough Variant Asthma and started Foracort 400 Rotacaps for 1 month twice daily with Bilasure 20 for 10 days. Symptoms are better and magically improves after foracort. Kindly guide on the diagnosis.

Your symptom pattern — cough worsening on lying down, after meals, talking, and laughing, with a sensation of incomplete breath and air hunger — is very suggestive of Laryngopharyngeal Reflux (LPR) or GERD-related cough, possibly alongside functional breathing pattern disorder. Normal PFT and borderline FeNO make classic asthma less likely. A trial of anti-reflux therapy and evaluation by a speech therapist for breathing retraining may be more helpful than just inhaler therapy.
Next Steps
Ask your pulmonologist about a GERD/LPR trial and consider a gastroenterology consultation. Breathing retraining with a physiotherapist can also help. Please consult me directly on Practo for a detailed evaluation.
