It seems like a simple case of Trapezitis.it's just tightening of muscles due to muscular imbalance. It happens as a result of a bad posture at work / home on overuse of desktop / laptops / cellphones and  using multiple pillows while sleeping. This problem has gradually risen over the years because of this bad posture. At times you might have headaches, vertigo and shoulder pain as well Though this problem is very simple to solve, it is very irritating and tiring, physically and mentally for the person involved. Normally in my answers I suggest home remedies first but since you have a chronic problem you need to go see a physio.
Next Steps
Physiotherapy treatment : SWD & ultrasonic therapy to loosen the muscles and you might require cervical traction as well. Treatment will take 15-20 days.
Health Tips
-follow neck ergonomics -take hot pack at home even though you have started with physio You can come & meet me at Aarogyam multispeciality clinic, Andheri & Kandivali, Mumbai

A neck pain can be indicative of a lot of conditions. Most commonly it arises due to bad posture, muscle imbalances and lifestyle habits like phone/computer usage.
Next Steps
Look up videos of neck stretches and perform them twice a day. Apply hot pack at the back of neck twice a day for 15 minutes. Look up photos/videos of good neck posture and practice it. Limit your phone usage.
Health Tips
If there is associated numbness or tingling in hands, or vertigo you must consult a Physiotherapist in person so that a thorough assessment can be done.
