Mouth open while sleeping

My son is 11 month old, formula feeding from month 2. His mouth is open While he sleeps , breathing through mouth I guess . Also during sleep at night , he woked up every 2 hours once and need us for sleeping with our shoulder . Kindly please advice , what we needs to do for him . I have attached his picture while sleeping .
