Hi it looks that you are suffering from Acute on Chronic disc prolapse L4L5 and probably it looks that you have DESSICATED disc prolapse and moreover it looks foraminal stenosis which at times can very painful and restricted range of movements , change of posture is must every half an hour to avoid fatigueness of the muscles and unless I see MRI plates it would be unfair to suggest whether you require surgery or not and now a days you can operate with MIS lumbar decompression with sos fusion and regular core group of back strenthing helps most of the time . Surgery can be decided only after seeing your MRI plates and clinically investigating you sir And if all the conservative methods of treating fails then we need to do MIS LUMBAR DECOMPRESSION WITH SOS FUSION WITH PEDICLE SCREW AND ROD ( MIS meaning minimal invasive technique.
Next Steps
Do an X ray LS SPINE AP AND LATERAL AND LATERAL VIEW IN FLEXION AND EXTENSION TO RULE OUT LYSIS I.E. break in pars interarticularis OR INSTABILITY IN LUMBAR SPINE
Health Tips
Start taking Physio IFT ( interferential currents ) and gradual back strenthing core group exercises. Take rest for 3 weeks and avoid lifting heavy weights Take some muscle relaxants after consulting your orthopaedic surgeon
