Kshar sutra for fissure

I am having tight anal sphincter which is very painful and bleeding. I have done sigmoidoscopy where is showed nothing. But my ayurvedic surgeon told me I have some internal pile mass to remove using kshar sutra. Is it painfull for internal mass removal using kshar sutra ? I am much worried about pain because I see lot of people say it's so painfull in Google reviews
