Itching and boil type thing

There is itching sometimes in hand and sometimes in legs...and mostly the itching is in testes region...there are boil type of things in my testes region and on top of the foreskin too...i am attaching picture... If this can be treated online than i am ready to consult or do i need in clinic consultation? Please check and help me... I am irritated with this itching... Candid dusting powder is not working on it... And luliac cream too not working...
