I have female pattern hair loss which is very severe from past one month which is bothering me a lot. I workout and eat good food. I have pcos, but my periods are regular with 30-40 days cycle.

The commonest cause for hair loss is iron deficiency anemia followed by folic acid deficiency. Since u have hypothyroidism with TSH of 4.5 which is normal but not under control. Ideally it should be between 1-2 if u have symptoms suggestive of hypothyroidism after ruling out other causes. Check Anti-TPO antibody too. If it is positive then u should consult a physician or Rheumatologist to rule out other causes of excessive hair loss. Autoimmune hypothyroidism is associated with many other autoimmune diseases. Consult a physician to find out cause for excess hair loss and its link with other problems.
