Treatment with insulin is only indicated when there are signs of insulin deficiency like excess urination, thirst weight loss etc or if hba1c is more than 9, consult for further detailed evaluation and treatment

A type 2 diabetic patient requires insulin if
1. the blood sugars are persistently high (F>140, PP>200, Hba1c>9) despite maximal dosages of oral medications
2. Patient is actively losing weight with symptoms like excessive urination
3. Oral medications are unsafe due to kidney or liver disorders
