A type 2 diabetic patient requires insulin if
1. the blood sugars are persistently high (F>140, PP>200, Hba1c>9) despite maximal dosages of oral medications
2. Patient is actively losing weight with symptoms like excessive urination
3. Oral medications are unsafe due to kidney or liver disorders
