The answer lies in 1. Techniques 2. Expertise 3. Foetal development/ law of land. Previously only four chamber view was enough to tell foetal heart is ok. That is also seen in your scan. But with new machines, new techniques and expertise now you can see tiny vessels, chamber abnormality. But it consume time and require high resolution machines and expertise so it cost extra. About timing, by MTP law, if there is defect in foetus you can terminate upto 20 wks with one consultant opinion, upto 24 wks with two consultants opinion and after that you have to apply district forum for approval even baby has problems (congenital anomaly). It is now considered among Indian expert that between 22-24 wks foetal heart mostly developed. So foetal echo is advised separately nowadays. So its patients decision and doctors advise. Only if it not done doctor will put into your record ‘advised but not done.’
Next Steps
Your decision to do or not to do.

Fetal echo is a detailed study of of baby's heart. Four chambered heart view done in tiffa scan is enough for major anomalies of heart . But for any minor defects 2d fetal echo is done . It is your decision if you wish to get it done.

As long as you dont have any risk factors like consanguineous marriage, elderly age, previous abnormality in report, history of abortions, etc,, No need to do... Opinion may differ from consultant to consultant..better to follow advice of your doctor
