Hi from the prescription slip attached the findings mentioned are that you have tooth decay in four teeth the numbers mentioned being the denotation of the tooth. As for the tooth numbered 8 is causing pain as its trying to pierce your gums and come out of the jaw . Hence indicated for extraction. Please attach the opg for further assistance.

As it's mentioned in your prescription that u have pain in your right lower back tooth region i.e 48 ( i.e 8th tooth /last tooth from centre ,4 represent quadrant )according to Dr .Suraj R sir's prescription.. That is wisdom teeth which is causing you pain and Dr has advised you to remove it.. Coming to other teeth which are addressed as   6/7/7 these teeth have small cavities and they need to be filled. This is additional awareness,when you visit any dental surgeon,we look into minor details and inform the patient what other issues the patient has and how to get them treated,and we explain ways to  prevent  them from further damage ,so that the patient is aware...
