In long-standing ankylosing spondylitis, hip range of motion can remain limited even after THR due to pre-existing soft tissue stiffness, muscle contractures, and spinalâpelvic rigidity caused by the disease itself. In cases with protrusio deformity and bone grafting, postoperative stiffness can be more pronounced because of altered hip biomechanics and prolonged healing. scar tissue formation and cautious rehabilitation to protect the graft may also restrict movement. While pain relief is usually good, full ROM is not always achievable in such complex AS hips.
