I have observed that my hair has been falling.whenever my run my hands through hair 4-5 hair will come out and hair density has also become very low and it's noticeable

From the symptoms you described it appears you to have androgenic alopecia which responds well to different treatments.  You need to  get Treatment that reduces the dht.

You are suffering from androgenic alopecia which is also called as pattern baldness. The hairs start reducing in density at the crown (top) and also the hairline starts receeding  from the front.
