Platelet count of 45,000–90,000/µL during pregnancy needs proper evaluation, especially because it was present from the first trimester. Since gestational thrombocytopenia usually appears later in pregnancy, an early and persistent low platelet count makes conditions such as immune thrombocytopenia (ITP) important to rule out.
At 4 months, I would advise assessment by an obstetrician and a hematologist. The treatment depends on the current platelet count, bleeding symptoms and the underlying cause. Not every patient with 45–90k needs medication immediately.
Next Steps
* Repeat CBC + peripheral smear
*  Check BP,
LFT, RFT, urine protein
*  Consult obstetrician + hematologist
*  Continue regular platelet monitoring.
Health Tips
* Avoid aspirin/ibuprofen unless prescribed.
*  Avoid unnecessary injections/trauma.
* Watch for unusual bruising or bleeding.
* Any significant bleeding, severe headache, or high BP → go to hospital immediately.
